Can nurses wear press-on nails? Bare below the elbows explained for the UK

Your last twelve-hour shift finishes on Saturday night, and by Sunday afternoon you're getting ready for a friend's wedding. Your own nails are short, dry from all the hand washing and gloves, and not quite what you had in mind for a day like that. On Monday morning you're back on the ward. That's usually when the question lands in our inbox.

So, can you wear press-on nails as a nurse or healthcare assistant in the UK? Not on duty, ever. Press-on nails are false nails, and false nails and nail polish have no place in patient care. On your days off, though, they're a very good fit: on in about ten minutes, off again in a few minutes with warm soapy water, and your own nails back to short and bare before your next shift.

The short answer

On duty: no. That includes short, neat, nude sets. Length and colour make no difference, because the guidance is about false nails and polish as such.

Days off: yes. A wedding, a birthday, a week's leave: that's when press-on nails come into their own. Put them on after your last shift and take them off before the next one. Your own trust's or employer's uniform policy has the final say on what you wear at work, so do read it.

What "bare below the elbows" means, and why it matters

Bare below the elbows is shorthand for keeping your hands, wrists and forearms free of anything that gets in the way of good hand hygiene. NICE guideline CG139 (recommendation 1.1.2.3) says healthcare workers should ensure their hands can be decontaminated by "being bare below the elbow when delivering direct patient care", "removing wrist and hand jewellery" and "making sure that fingernails are short, clean and free of nail polish".

The epic3 guidelines for preventing infections in NHS hospitals in England go a step further on nails. Recommendation SP8 includes "making sure that fingernails are short, clean, and free from false nails and nail polish". False nails are named outright, not just polish.

What the guidance says about nails in England, Scotland, Wales and Northern Ireland

England: NHS England and NHS Improvement's 2020 guidance for employers gives examples of good and poor practice. Under good practice: "Have clean, short, unvarnished fingernails." Under poor practice: "Wear false nails during patient care activity." On jewellery, it says: "Local policies may allow a single plain metal ring, such as a wedding ring." The guidance leaves uniform policy to each organisation, so your own trust's dress code is the one that applies to you.

Scotland: the hand hygiene literature review in the National Infection Prevention and Control Manual notes: "DL(2018)04 also states that NHSScotland HCWs should not wear false/artificial nails when providing patient care." It adds: "The NICE and epic3 guidelines recommend that fingernails of HCWs should be free of nail polish when delivering patient care."

Wales: the All Wales NHS Dress Code couldn't be much clearer: "Clinical staff must not wear false nails or nail varnish". It also says: "All clinical staff must wear short sleeves or elbow-length sleeves in the workplace to enable effective hand washing techniques."

Northern Ireland: the Regional Infection Prevention and Control Manual is just as direct: "Keep your nails short and refrain from wearing artificial nails, nail extensions, gel nails, or nail varnish/polish."

Why press-on nails count too

We make press-on nails, and we'd love to tell you otherwise, but they are false nails. Glue-on or held with adhesive tabs, they sit in the same category as any other false nail. Anything on top of your own nail makes hand hygiene harder, and an edge that starts to lift is harder to keep clean underneath than a bare nail, however careful you are. That's why the guidance names false nails and polish specifically. They simply belong to your own time, not to a shift.

Other jobs with nail rules

We also hear from dental nurses, care home staff and nursery workers. To be straight with you, we found no single national rule on nails for those settings that we could verify. Your employer's infection control or hygiene policy decides, and wherever you give clinical care, the same principles apply. If you work with food, the Food Standards Agency's Safer Food, Better Business guidance says: "Staff should not wear watches or jewellery when preparing food (except a plain wedding band)." The FSA documents we read don't mention nails, so ask your employer.

How to wear press-on nails around your shifts

  • Put them on after your last shift, once you're home and your uniform's in the wash.
  • Take them off the evening before your next shift, so you're not rushing before an early start.
  • Soak them in warm soapy water for a few minutes and lift gently from the side. No acetone, no filing.
  • Keep your own nails short and clean underneath, as the guidance asks.
  • Wash off any leftover tab residue, so your nails really are bare when you're back on duty.

Questions nurses ask us

Can nurses in the UK wear press-on nails?

Not on duty. Press-on nails are false nails, and the guidance in England, Scotland, Wales and Northern Ireland is clear that false nails aren't worn during patient care. On your days off, they're yours to enjoy.

Is short gel polish allowed on the ward?

No. Gel polish is still polish, and the guidance asks for nails free of polish or varnish during patient care. Northern Ireland's manual names gel nails outright. However short or pale it is, it stays off on shift.

Does this apply to HCAs, carers and student nurses?

Yes. The guidance talks about healthcare workers and clinical staff giving patient care rather than specific job titles, so if you're an HCA, a midwife or a student nurse on placement, it's written for you too. If you're a carer outside the NHS, your employer's infection control policy decides, and the same principles apply.

Can I wear press-on nails for one day off between shifts?

Yes, and it's one of the things they're best at. Put them on after your shift and take them off the evening before you go back. Soaking takes a few minutes, so your own nails are short and bare again in good time.

Do press-on nails damage your own nails?

Most of the damage people worry about comes from how nails are taken off, so here's how ours should come off. Soak them in warm soapy water for a few minutes and lift gently from the side, with no acetone and no filing. If one is still holding on, give it a little longer in the water rather than pulling.
