Nurses and press-on nails in Canada: what the hand hygiene rules say, and how to wear them on your days off

If you work in patient care in Canada, you've probably already had this conversation with yourself: you love a fresh set of press-on nails, but your unit has rules about nails at work. The good news is you don't have to choose between the two. You just have to be honest about what press-on nails actually are, know what the hand hygiene guidance says, and plan your sets around your shifts instead of wearing them through them. Below you'll find what the guidance says, and how nurses and other care workers wear press-on nails on their days off.

Why press-on nails aren't worn during patient care

This isn't a L'Adele house rule. It comes from the infection prevention and control guidance that Canadian healthcare workplaces follow. IPAC Canada, the national body for infection prevention and control, puts it plainly in its hand hygiene practice recommendations:

"Health care workers (HCW) providing patient care and others should not wear artificial nails or nail enhancements/nail art, including but not limited to acrylic gel nails and wraps. These have been associated with increased glove tears, transfer of microorganisms, and outbreaks of infection."

In Alberta, the provincial health authority spells out the same standard for its own staff:

"Artificial nails and/or nail enhancements (e.g., fibreglass or silk wraps, decals, rhinestones, gel polish), shall not be worn by AHS peoples who provide direct patient care."

The reasoning is the same in both: artificial nails of every kind, whether acrylic, gel, a wrap or a press-on, are linked to more glove tears and to microorganisms being passed on. In a clinical setting that's a real infection prevention issue, and it's not something we can talk our way around.

Let's be honest about what counts

Press-on nails are artificial nails. There is no version of "but mine are different" that changes that, no matter how thin, soft gel, or well made a set is. If a policy says no artificial nails or nail enhancements for anyone providing direct patient care, that covers press-ons too, alongside acrylics, hard gel, and dip powder. We would rather tell you this clearly than have you assume press-ons are exempt and end up in an awkward conversation with your manager or your infection control team.

The exact rule depends on your province and your employer

Hand hygiene guidance in Canada is published nationally by bodies like IPAC Canada and the Public Health Agency of Canada, but it gets implemented locally. Every province, health authority, hospital, and clinic writes its own policy on top of that guidance, and the details can differ, including which roles are covered and how nail polish is treated. Before you plan anything around your own nails, check your own workplace policy or ask your infection prevention and control lead. How to wear press-on nails around your shifts

None of this means press-on nails are off the table for you; it just means the timing matters. Here's how to make it work.

Put them on after your last shift

In between, keep your own nails short and clean, exactly as the hand hygiene guidance asks.

How to measure your nail

  1. Measure the widest part of your nail, from side to side.
  2. No measuring tape? Wrap a strip of paper around your nail, mark both edges and measure the strip flat with a ruler.
  3. Note the width in millimetres and look up your size in the chart.
