Can Dental Assistants and Dental Hygienists Wear Press-On Nails?

If you work chairside as a dental assistant or dental hygienist, press-on nails fall into the same category as any other artificial nail for infection control purposes, so they are not something to wear while you are treating patients. That does not mean they have no place in your routine. A well-made set still works beautifully on your days off, and it comes off in minutes with nothing but warm water when your next shift comes around.

Why press-ons count as an "artificial nail"

The CDC's Guidelines for Infection Control in Dental Health-Care Settings groups artificial fingernails and extenders together when it discusses hand hygiene, and the reasoning has nothing to do with brand or price. It comes down to what sits between your glove and your skin. The guidance states plainly: "Hand carriage of gram-negative organisms has been determined to be greater among wearers of artificial nails than among nonwearers." It also notes: "Artificial fingernails or extenders have been epidemiologically implicated in multiple outbreaks involving fungal and bacterial infections in hospital intensive-care units and operating rooms."

A press-on set is handmade and applied at home rather than built up in a salon, but physically it does the same thing an acrylic or gel extension does: it adds a shaped layer over your natural nail, with an edge near your cuticle and a longer tip that your gloves have to fit over. For infection control, that makes it an artificial nail, and the same rule applies.

The glove problem, not just the germ problem

Gloves are the other half of this. The same CDC guidance says fingernails "should be short enough to allow DHCP to thoroughly clean underneath them and prevent glove tears," and separately warns that "long artificial or natural nails can make donning gloves more difficult and can cause gloves to tear more readily." A torn glove mid-procedure is a much bigger problem than a chipped manicure, and it is the practical reason many offices do not allow any nail extension, press-ons included, for staff who glove up and treat patients.

Nail polish is treated differently in the same guidance. Freshly applied polish on short, natural nails "does not increase the microbial load from periungual skin if fingernails are short," though it adds that "chipped nail polish can harbor added bacteria." Polish and press-ons are not the same question, which is why some offices allow one and not the other. Your practice's own policy decides this for you, always.

Days off are a different story

Away from the operatory, the workplace rules no longer apply, and a press-on set is an easy way to have beautiful nails for a weekend, a wedding or a few days between shifts. Frequently asked questions

What about front desk staff or office managers who do not touch patients?

The CDC guidance above is written specifically for dental health care personnel who provide direct patient care and glove up for it. Front desk and administrative staff who never glove or work chairside are not covered by the same reasoning, though plenty of practices still set one dress code for everyone in the building. Check with your office manager or practice policy rather than assuming either way.

Can dental students wear press-on nails in a school clinic?

Dental and hygiene school clinics generally follow the same infection control standards as a working practice, since students are gloving up and treating patients under supervision. Most programs publish their own clinic dress and grooming policy, and that document, not a general guideline, is what applies to you day to day.

Can I wear nail polish instead, if press-ons are not allowed on shift?

That depends entirely on your practice's policy, since polish and artificial nails are treated as separate questions in the CDC guidance itself. Some offices allow short, unchipped polish and simply prohibit any nail extension or enhancement. Ask what your specific office allows rather than assuming polish is automatically fine.

Will wearing press-ons on my days off damage my natural nails?
